Approves member surveys, reviews Care Manager Activity notes and manages ticklers

Manages engagement, enrollments, non-enrollments and discharges of members in assigned branches

Establish or adopt policies / procedures for, and keep records of criteria for admission to services, case assignments and case management

Establish a process to verify that skilled nursing and personal care services were provided

Ensures client adheres to discharge plan and monitors client for key issues to avoid re-hospitalization

• 4) A Bachelor's Degree in Administration with a concentration of study in health services administration nd at least 2 years supervisory or administration experience in a health care facility/program which included care of the sick OR
• 5) Employment as the administratior of a home health care agency in the state of Connecticut as of January 1, 1981 who has been so employed continuously for the five (5) years immediately preceding January 1, 1981 OR
• 6) Continuous employment as an administrator of a home health care agency as of January 1, 1979
• 2-3 years of experience in home care or working with the elderly population
• Licensed Registered Nurse, preferred but not required

• Must be able to be in the field at least 4 days/week to include 1-2 overnights/week
• Must be a licensed Registered Nurse in the state of Ohio
• Previous management experience in a one-to-one home care setting preferred
• Outside sales experience in the health care field preferred
• 2-5 years of hands-on supervisory experience

• Must be a licensed Registered Nurse in the state of Connecticut
